How to Declare the ESRM Major
Request Form for Declaring the ESRM Major
Verify you have completed the Minimum Admission Requirements for the ESRM Major. Check one of these for more information, if you haven’t already done so:
- Admissions Information if you are a current UW student, or
- Admissions Information if you are a prospective Transfer Student or a new Transfer admit to the UW who has not gone through New Student Orientation yet.
I’ve completed the Minimum Admission Requirements and am ready to declare the ESRM Major. Now what?
How to Declare the Major
The declaration form consists of a brief list of questions (found below) to answer, and the declaration form should be completed in one sitting. You must be signed in to your UW NetID account to access this form. Students must be current UW students (you may fill out and submit the form as early as your New Student Orientation, assuming you have completed all the required courses and they are recorded on your UW student record.

Questions to answer and knowledge needed for the Declaration form
- Your Personal Statement, answering the following questions in approximately 250 words total:
- Why do you want to be an ESRM Major?
- How will completing this degree help you complete your professional and academic goals?
- Your UW Student Number, your UW Email address, and how you first learned about the ESRM major.
- Review each of our 5 major pathways for completing the ESRM degree; these are listed on the left side-menu.
- Download and save a copy of the ESRM: General Degree audit.
Are there reasons why I might not be eligible to declare the major?
As mentioned above, students cannot declare the major until they are a current UW student, and new admits must wait until any courses taken at other colleges/universities are recorded on their UW transcript; this recording occurs right before the student’s New Student Orientation.
There are other reasons why a student might be ineligible to declare the ESRM major at this time, despite having completed the Minimum Admission Requirements:
- Students must have completed all of the Minimum Requirements; requirements in progress are not accepted. If there is a reason why you must submit the declaration form while some requirements are listed as in progress, please contact SEFS Advising.
- Students currently on Academic Alert, Academic Warning, or Academic Drop are ineligible to declare the major until these are resolved. Please consult your unofficial UW Transcript to see if one of these statuses is posted for your most current completed quarter.
- Your student record must also indicate that you have no outstanding admission deficiencies (if you have them, you will see these listed near the top of your ESRM: General degree audit, and they are also listed at the top of your UW Transcript).
- Students must be able to finish an ESRM degree within UW Satisfactory Progress rules.
- Students who are applying to ESRM as a second or third major/degree should also be aware of the UW Financial Aid rules regarding Satisfactory Academic Progress.
